If you are a new patient interested in an initial consultation or getting on the waitlist, the form below is the easiest way to contact me. Online forms are not secure, so please understand that your privacy cannot be ensured using that method, and email is also an option (drkim@kimdavisphd.com). Please allow 24-48 hours for a return phone call or email. I only schedule sessions between the hours of 9 am and 3 pm, Monday through Friday.

My contract with Blue Cross insurance ended January 2021. I am an out-of-network provider for all insurance companies and can provide a receipt for my services to submit to your insurance company for possible reimbursement.
